About the Role:

Join Sunrise Health Care, PC as a Family Nurse Practitioner (FNP-C) in beautiful Colorado Springs, CO! We are looking for a passionate and dedicated healthcare professional to provide exceptional patient care in a supportive and collaborative environment.

Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans.
• Perform diagnostic tests and interpret results to guide treatment decisions.
• Administer medications and provide patient education on health management.
• Collaborate with multidisciplinary teams to ensure holistic patient care.
• Maintain accurate and timely documentation of patient interactions.
• Stay current with best practices and contribute to quality improvement initiatives.
• Participate in community outreach programs to promote health awareness.
• Provide compassionate support and guidance to patients and their families.

Requirements:

• Current FNP-C certification and a valid Colorado nursing license.
• Minimum of 3 years of clinical experience in primary care family practice or a related field.
• Strong communication skills and a patient-centered approach.
• Ability to work effectively in a fast-paced environment.
• Commitment to continuous learning and professional development.
• Experience with electronic health records (EHR) systems. EPIC experience helpful.
• Demonstrated ability to build rapport with diverse patient populations.
• Willingness to participate in team-based care and community initiatives.
